NHS Forth Valley offer these travel assessment and vaccination for travellers planning to visit areas of medium or high risk of disease. We ask travellers to contact us via the Travel Assessment Form at least 6-8 weeks before departure.  Patients who can't use the eform should contact the immunisation team on 0800 130 3120.
